Job Summary
Coordinate outreach and housing placement for residents in Permanent Supportive Housing as part of the FSP Housing First team. Provide intensive case management, outreach and engagement, intake and assessment, housing applications, benefits establishment, and lease-up support. Collaborate with clinical resources and co-service providers to develop and monitor Personal Service Plans, using recovery-oriented models (Housing First, Harm Reduction) to secure stable housing. Conduct regular home visits, monitor for decompensation, facilitate life skills activities, and ensure access to psychiatric and medical resources. Maintain documentation in Electronic Health Record (Welligent) and meet billing quotas, with 24/7 on-call coverage as required. Requires a BA or equivalent experience and ability to work within a multidisciplinary team.
Required Qualifications
- Bachelor’s degree in psychology/social work or 2 years of related work experience working with homeless populations and/or those diagnosed with a mental illness in a community setting desirable
