FPS Case Manager

Job Summary
Serve as the primary point of contact for families receiving autism treatment services by conducting intake interviews, gathering family needs, and coordinating care among behavior analysts, therapists, physicians, schools, and community agencies. Connect families with resources, support groups, and financial assistance while advocating for their needs during treatment planning and interdisciplinary meetings. Monitor client progress, maintain accurate documentation in compliance with HIPAA, and participate in case reviews and quality improvement initiatives. Provide parent education, coaching, and support related to behavior management and service navigation. Assist with scheduling appointments, referrals, and authorizations, and facilitate crisis intervention referrals when necessary.
Required Qualifications
- Licensed Childcare Administrator Certification
- Minimum 1–2 years of experience working with children with autism, developmental disabilities, behavioral health services, or family support programs
- Ability to travel locally for home visits, school meetings, and community appointments as needed
- Ability to sit, stand, and use office equipment for extended periods
Desired Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Special Education, or a related field
- Experience in case management, family advocacy, or community resource coordination
- Knowledge of Autism Spectrum Disorder (ASD) and evidence-based treatment approaches
- Knowledge of community resources and support systems
