Job Summary
Provide case management, mental health services, and transportation to youth and families, meeting established outcome thresholds. Coordinate clinical services across medical, educational, and social domains while regularly reviewing and modifying youth-based plans. Establish relationships with foster parents and assist in reunification or stabilization efforts consistent with case plans. Deliver safety assessments, crisis response, and behavioral stabilization as required. Achieve billable productivity thresholds and complete all documentation and training timely. Travel daily to deliver community-based and in-home services in compliance with regulatory requirements. Maintain a valid driver's license and reliable transportation.
Required Qualifications
- Bachelor's degree in Social Work or comparable Human Services field from an accredited institution or equivalent
- A valid professional license (LSW) as required by state law and/or regulatory bodies
- A willingness to work flexible and non-traditional hours as needed in the service of foster caregivers, families of origin, and persons served
- Capacity to be flexible and responsive to youth served, foster caregivers, system partners, and internal customers
- Capacity to remain objective and professional in all areas of job function
- Demonstrates tolerance and respect for ideas and actions of others
- Capacity to effectively work with and be respectful and sensitive to persons from various cultures, socioeconomic, ethnic, religious, and racial backgrounds
- Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs
- Valid driver's license
- Reliable personal transportation
- Good driving record
- Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability
