Job Summary
Conduct family assessments to identify strengths, needs, and problem areas, utilizing treatment modalities to effect growth and change. Coordinate comprehensive case management plans, develop alternative placement resources, and complete discharge summaries eight weeks prior to release. Deliver case management services by scheduling counseling, ensuring medical and dental examinations, and maintaining safe environments through bi-monthly home and community visits. Act as a bridge between youth, families, and treatment teams via conferences and reviews, while assisting with reunification by identifying mentors, recreational opportunities, and relapse prevention programs. Maintain accurate documentation, ensure confidentiality compliance, and manage transportation for court and appointment obligations. Requires a Bachelor's degree in Human Services or Social Work, valid driver's license, and ability to work flexible evening and on-call schedules.
Required Qualifications
- Bachelor's degree in Human Services, Social Work, Psychology, or a related field
- Valid driver's license with a driving record acceptable to agency standards
- Access to a reliable vehicle
- Ability to plan and coordinate transportation for home visits, appointments, and court-related obligations in an efficient and timely manner
- Must be able to work a flexible schedule that includes variable hours: days, evenings and occasional on-call responsibilities
Desired Qualifications
- Master's degree (MSW, MHC, MFT, or related)
- 1 years of direct experience working with at-risk youth and families, case management, child welfare, trauma-informed care preferably in a home-based, or community setting
- Ability to engage and support individuals and families from economically and culturally diverse backgrounds
- Demonstrated understanding of, equity, and inclusive practices in service delivery
- Excellent communication skills, including clear and professional verbal interactions, effective and empathetic listening, and accurate, objective, and timely written documentation
- Proficient in Microsoft Office and web-based documentation systems
