Job Summary
Conduct comprehensive case management plans for youth and families to ensure safety, permanency, and well-being. Assess family strengths, needs, and problem areas while utilizing treatment modalities to effect growth. Deliver case management services including scheduling counseling, coordinating medical and dental examinations, and ensuring compliance with confidentiality standards. Maintain safe environments by engaging youth and families through home and community visits, at least twice monthly, to focus on treatment goals. Act as a bridge between youth, families, and treatment teams via telephone, face-to-face contacts, and comprehensive treatment reviews. Assist with reunification by implementing permanent supports such as mentorship, recreational opportunities, and relapse prevention programs. Complete discharge summaries eight weeks prior to youth discharge. Work flexible hours including evenings and occasional on-call responsibilities, utilizing a valid driver's license to coordinate transportation for home visits and court obligations.
Required Qualifications
- Bachelor's degree in Human Services, Social Work, Psychology, or a related field
- Valid driver's license with a driving record acceptable to agency standards
- Access to a reliable vehicle
- Ability to plan and coordinate transportation for home visits, appointments, and court-related obligations in an efficient and timely manner
- Ability to engage and support individuals and families from economically and culturally diverse backgrounds
- Demonstrated understanding of, equity, and inclusive practices in service delivery
- Excellent communication skills, including clear and professional verbal interactions, effective and empathetic listening, and accurate, objective, and timely written documentation
- Proficient in Microsoft Office and web-based documentation systems
- Must be able to work a flexible schedule that includes variable hours: days, evenings and occasional on-call responsibilities
Desired Qualifications
- Master's degree (MSW, MHC, MFT, or related)
- 1 years of direct experience working with at-risk youth and families, case management, child welfare, trauma-informed care preferably in a home-based, or community setting
