Foster Care Coordinator - TN

Conduct regular home visits to foster families to monitor children's well-being and ensure service needs are met. Engage birth parents to advance permanency plans and provide crisis intervention when necessary. Serve as active therapeutic mentors and teachers, attending case planning meetings and court hearings. Create treatment plans, coordinate scheduling for health and educational services, and assist older teens with independent living skills. Document all client contacts and service interactions while maintaining excellent communication with stakeholders. Requires a Bachelor's degree in Social Work or related field, valid driver's license, and minimum 21 years of age.
