Foster Care Case Planner

Case Planners will receive specific training in evidence-based and evidenced-informed interventions that will assist the Case Planner support families and caregivers reach permanency goals within a reasonable period of time. S/he will be responsible for planning for families with the intent of achieving a permanency for children either through reunification with birth parents/discharge resources, and if not possible, through adoption, KinGap or APPLA. Primary Responsibilities include conducting field visits to foster homes and birth parent's homes, coordinating and supervising family visits, facilitating weekly groups for birth parents and foster parents, ensuring each case adheres to ACS’ permanency timeline ASFA guidelines, maintaining case compliance via data entry and record keeping, accompanying families to necessary service appointments, attending trainings and court hearings, and participating in family team conferences to present at weekly case conferences. Minimum Requirements include a Bachelor's or Master's degree in social work, psychology, criminal justice or counseling, ability to facilitate groups, cultural competence, strong communication, flexibility in schedule, travel within NYC region, and language skills (Spanish/Mandarin/Cantonese/Russian a plus).
