Job Summary
Foster Care Case Manager (FCCM) is responsible for safety and services to children and families in foster care, including facilitating Family Support Team meetings, development of individualized care (treatment) plans, coordinating services across home, school, and community settings, and documenting support activities. Maintains a caseload of 12 to 15 children, conducts monthly face-to-face contact with each child and family, and participates in agency on-call rotations. Requires driving transportation, travel to various community locations, and compliance with multi-step screenings and certifications. Must have a bachelor’s degree in a relevant field (master’s preferred) and at least two years of relevant case management experience. Must hold a valid driver’s license and be willing to undergo background checks, drug screen, and pre-employment medical and safety screenings. CPR/First Aid certification is required. The role supports flexible work arrangements (office/home) and involves frequent documentation in state databases (FACES, CSWIS) and coordination with service providers and judicial processes.
