Job Summary
Provide case management and counseling services to an assigned individual caseload, assessing referred individuals and leading the development, coordination, and evaluation of service plans with treatment teams and stakeholders. Maintain current and complete records adhering to corporate, contractual, and regulatory standards while providing ongoing monitoring and consultation to mentors regarding service plan implementation. Conduct mentor and family home visits, lead mentor group meetings, and assist with emergency coverage and crisis intervention support through the rotational on-call system. Assist with legal, licensing, regulatory, and certification activities as needed. This role requires a Bachelor's degree in human services or related field, one year of experience working with individuals with developmental disabilities, and current licensure including CPR and First Aid. A reliable driver's license and car registration are required. Full-time employment includes a comprehensive benefits package with 401(k) matching, paid time off, and holiday pay.
