Foster Care Case Manager

Job Summary

Review referrals and match children with appropriate state licensed foster families. Place children in certified homes and obtain all required documentation. Maintain weekly contact with foster families, children, and bio-families while monitoring placement progress. Prepare individual treatment plans within 21 days of placement and quarterly thereafter, plus monthly progress reports. Provide Independent Living training, coordinate visitation with biological parents and relatives, and conduct supervised visits as required by court. Act as liaison with placing agencies, schools, and community partners. Attend court hearings, staff meetings, and training seminars. Complete interviews, training, and home studies for prospective foster parents. Maintain flexible hours, including weekends and holidays as needed.
