Job Summary
Coordinate case management services by reviewing referrals, matching children with licensed foster families, and placing youth in state-certified homes. Conduct weekly documented contact and bi-weekly visits to children and foster homes, while visiting assigned children monthly away from the foster setting. Develop individualized treatment plans within 21 days of placement and quarterly thereafter, preparing monthly progress reports and independent living progress reports. Provide supervision, training, and home studies for foster parents, coordinate visitation with biological parents and relatives, and attend court hearings and review hearings. Act as a liaison with placing agencies, schools, and community partners, and complete interviews and training as needed. Maintain flexible working hours, including weekends and holidays, to support family-centered practices and ensure safety, permanency, and stability for youth.
Required Qualifications
- Bachelor's degree in social work, sociology, psychology, counseling, or related human services field
- Two years' experience working with children and families
- Five years' experience with a placing agency working with troubled children and families (in lieu of a graduate degree)
