Job Summary
Independently Licensed Therapists (LICSW, LMFT, LPC) sought to join a SNF Therapy team in Colorado. Responsibilities include completing diagnostic evaluations, psychotherapy, treatment planning, and in-person consultations at partner SNF/LTC facilities; collaborate with BHS psychiatrists, psychologists, care coordinators, and facility staff; document remotely in Athena I EMR; comply with agency policies. Requires a Master’s degree in Social Work or Counseling and Colorado licensure (LCSW/LMFT/LPC), NPI, and Medicaid/Medicare credentialing is a plus. Required Qualifications
- Master of Social Work, Counseling, or other eligible master's degree
- Colorado licensure as an LCSW, LMFT, or LPC
- NPI (National Provider Identifier)
- Credentialed with Medicaid/Medicare (plus)
- TB Test
- CPR/AED/First Aid Certification
- Fluency in Spanish is a plus
