Job Summary
Forensic Case Manager role focused on coordinating care for justice-involved clients. Responsibilities include conducting intake assessments with psychosocial histories and risk/needs evaluations; developing individualized case management and recovery plans aligned with legal requirements and client goals; monitoring compliance with court orders and treatment engagement; facilitating access to behavioral health services, housing, employment, and education resources; maintaining ongoing communication with probation officers, attorneys, judges, treatment providers, and other stakeholders; providing crisis intervention and advocacy; documenting activities per agency policies, HIPAA, and legal standards; participating in multidisciplinary team meetings and court proceedings; tracking outcomes for program evaluation and quality improvement. Required Qualifications
- Bachelor’s degree in Social Work, Criminal Justice, Psychology, Counseling, or related field required; Master’s degree preferred.
- Minimum of 2 years’ experience in forensic case management, probation services, or behavioral health.
- Knowledge of criminal justice processes, behavioral health systems, and evidence-based practices in recovery.
- Strong interpersonal and communication skills; ability to engage individuals from diverse backgrounds.
- Ability to manage a caseload with competing priorities in a fast-paced environment.
- Proficiency in documentation, case notes, and electronic health records.
