Job Summary
The provider will join a two-person clinical team to establish a new urgent care in Middletown, delivering patient-centered care for acute illnesses and minor injuries across the lifespan. The role supports independent practice within a mission-driven, nonprofit FQHC setting, offering a 4–10 hour workday schedule with rotating Saturdays. Required qualifications include NYS licensure as a Family Nurse Practitioner or Physician Assistant, active DEA, board certification/eligibility, BLS/CPR, and a minimum of 3 years of urgent care, emergency medicine, or primary care experience, plus strong clinical decision-making and procedural skills. Required Qualifications

- Active DEA
- Board Certified/Eligible
- Current AHA BLS/CPR
