Job Summary
Float Specialty Nurse Case Manager role provides advocacy, care coordination, and guidance for patients with complex medical and social needs across Dora Street and Lakeview Health Centers. Collaborates with multidisciplinary care teams to ensure high-quality, comprehensive care; conducts patient assessments, education, and action planning; supports treatment adherence, medication reconciliation, and follow-up care; coordinates referrals to community resources and specialty services to address medical, behavioral, social, and financial barriers; monitors labs and provider visits; travels between sites.
Required Qualifications
- Active California LVN or RN license
- Completion of an approved IV therapy course
- Current BLS (Basic Life Support) certification
- Willing and able to travel between sites
- Working knowledge of Electronic Medical Records use and process
