Job Summary
Conduct skilled nursing assessments, update care plans, and respond to urgent patient needs while collaborating with interdisciplinary teams. Administer and titrate medications, manage individual patient medication and residence contingency supplies, and coordinate volunteer deliveries. Provide education to patients and families regarding disease progression and symptom management, delegate nursing duties to LPNs, and supervise CNA care. Complete new patient admissions, perform telephone triage, and maintain accurate records of patient and physician communications in the Electronic Medical Record. Prioritize daily work to address multiple tasks and adjust to frequently changing demands. Full-time, 0.9 FTE (36 hours per week) with variable shifts between 6:00 a.m. and 6:30 p.m., including weekend rotations and on-call duties.
Required Qualifications
- Associate degree in Nursing
- RN license in the state of practice
- Valid WI Drivers' License
- Reliable transportation
- Proof of current automobile insurance coverage
- Computer proficiency (MS Office – Word, Excel, and Outlook)
- Ability to communicate effectively, both verbally and in writing
- Ability to maintain confidentiality and discretion in business relationships and exercise sound business judgment
- Ability to keep up to date with policies, practices, and regulations
- Ability to prioritize and handle multiple tasks and capable of dealing with deadline pressures
- Ability to adjust to frequently changing demands
