Job Summary
Examine, evaluate, and treat patients recovering from surgeries, strokes, orthopedic injuries, cardiac conditions, and neurological disorders across Gulf Coast Skilled Nursing Unit, Lee Memorial Skilled Nursing Unit, and HealthPark Care & Rehabilitation Center. Design and modify individualized treatment plans to restore mobility, function, and independence while collaborating with interdisciplinary medical teams. Manage patient caseloads that vary by location and complexity, ensuring safe transitions home for adult populations in a post-acute environment. Utilize EPIC for documentation and maintain Florida Physical Therapy licensure.
