Float Physical Therapist - Skilled Nursing (SNF/SNU)

Conduct physical therapy evaluations, testing, and treatment interventions for adult patients recovering from surgeries, strokes, orthopedic injuries, cardiac conditions, and neurological disorders. Design and modify individualized treatment plans to restore function, promote independence, and support safe transitions home across Gulf Coast SNU, Lee Memorial SNU, and HealthPark Care & Rehabilitation Center. Reduce injury risk and maintain fitness for diverse patient populations while collaborating with interdisciplinary teams in a 75-bed, 18-bed, and 112-bed facility environment. Utilize EPIC EMR for documentation and adhere to Florida licensure requirements.
