Float Physical Therapist - Skilled Nursing (SNF/SNU)

Examine, evaluate, and test individuals with mechanical, physiological, and developmental impairments to determine diagnoses, prognoses, and treatment plans. Design, implement, and modify interventions to alleviate functional limitations while reducing injury risks and promoting fitness across adult populations recovering from surgeries, strokes, cardiac conditions, and orthopedic injuries. Collaborate with interdisciplinary teams to develop individualized treatment plans that restore mobility and support safe transitions home within Gulf Coast, Lee Memorial, and HealthPark Skilled Nursing Units. Conduct ongoing assessments of intervention effects and engage in administration, consultation, and education. Utilize EPIC for documentation and apply critical thinking, problem-solving, and stress management skills in a float role between three Fort Myers facilities.
