Job Summary
Examine, evaluate, and test individuals with mechanical, physiological, and developmental impairments to determine diagnoses, prognoses, and treatment plans. Design, implement, and modify interventions to alleviate functional limitations while reducing injury risk and promoting fitness across adult populations recovering from surgeries, strokes, cardiac conditions, orthopedic injuries, and neurological disorders. Collaborate with interdisciplinary teams to develop individualized treatment plans that restore mobility and support safe transitions home in Gulf Coast, Lee Memorial, and HealthPark Skilled Nursing Units. Utilize EPIC for documentation and maintain professional standards in a float role providing diverse clinical experience across multiple post-acute care settings.
Required Qualifications
- Bachelors, Masters, or Doctor of Physical Therapy
- Physical Therapy License (State of Florida)
- EPIC EMR System experience

- Experience across multiple post-acute care settings (Gulf Coast SNU, Lee Memorial SNU, HealthPark Care & Rehabilitation Center)
- Experience with adult patients recovering from stroke, orthopedic procedures, joint replacements, cardiac and pulmonary conditions, and neurological disorders
