Job Summary
Conduct comprehensive assessments to determine patient needs and develop personalized treatment plans. Deliver 1:1 rehabilitative therapy to patients recovering from surgery, illness, or accidents within their homes or care facilities. Constantly evaluate evolving patient needs and adjust plans of care accordingly. Practice independently as part of an interdisciplinary care team while assessing progress by observing patients during their daily routines.
Required Qualifications
- New York State License and current registration in Physical Therapy
- Valid driver's license
- Bachelor's Degree in Physical Therapy from a program approved by the New York State Department of Education
- Minimum of one year physical therapy experience
Desired Qualifications

- successful completion of the probationary period as a VNS Health Rehab Intern
- Basic PC skills
- Bilingual skills
