Job Summary
Conduct comprehensive assessments of patients to determine needs and develop personalized rehabilitation treatment plans. Deliver 1:1 rehabilitative therapy to patients recovering from surgery, illness, or accident in their homes or care facilities. Constantly evaluate evolving patient needs and adjust plans of care accordingly. Practice independently in the community as part of an interdisciplinary care team. VNS Health, a 130-year nonprofit, helps New Yorkers heal in their own homes by connecting them with family and community resources.
Required Qualifications
- Minimum of one-year physical therapy experience
- Successful completion of probationary period as a VNS Health Rehab Intern
- New York State License and current registration in Physical Therapy
- Valid driver's license
