Job Summary
Provide continuity of care and caseload coverage during Peer Navigator leaves of absence, extended vacancies, and other staffing gaps in Anaheim and Garden Grove. Build rapport with members using trauma-informed engagement to coach strategies reducing risks for behavioral health, substance use, and housing instability. Connect members to medical, behavioral health, housing, and benefits services while maintaining accurate documentation across Epic, HMIS, and CalOptima systems. Safely operate the HIA medical van, including transporting the clinical team, cleaning the vehicle, and obtaining vital signs or performing phlebotomy if certified. Schedule encounters based on member acuity and work collaboratively with clinicians and Community Support Partners to ensure care coordination and follow-through.
Required Qualifications
- G.E.D./High School
- 3+ years of work experience
- Ability to work in a dynamic, outdoors environment
- Ability to work independently as well as part of a team
- Ability to organize effectively
- Ability to prioritize multiple and competing tasks
- Ability to communicate effectively, including articulating one's own relevant personal experiences
- Excellent oral communication skills

- Maintenance of reliable transportation
- Proficiency in operating a computer, including logging into and maneuvering around multiple systems such as online timesheets (time and attendance) and call center software

- 2 years of work experience
- Bachelor's degree
- 1-2 years of work experience
- direct work experience, lived experience, training or volunteer work with patients experiencing homelessness
- active mental health and substance use conditions
- Other demonstrated relevant experience in similar community settings, for example, working with other disadvantaged groups, and/or outreach
- A comparable combination of education/experience and/or training
- HMIS/ Coordinated Entry System
