Job Summary
Flex Team - Personal Care Partner II (QMAP) PRN provides clinical services to residents in assisted living settings, including ADLs, medication assistance, and documentation, on a flexible, as-needed basis with a minimum fill-in of 2 shifts per month; shifts are typically 12 hours and multiple locations may be utilized. Must hold medication certification (QMAP-A/L, Med Tech, Med Aide) and perform duties under supervision with attention to resident needs, medication administration per state law, and ongoing communication with supervisors; strong English literacy and prior elder-care experience preferred.
Required Qualifications
- High school or Equivalent (GED)
- Medication certification; QMAP-A/L, Med Tech, Med Aide, Nurse Delegation
- Must be able to read, write and speak English
- One (1) year experience in elder care preferred
