Job Summary
The Field Supervisor, In-Home Services oversees Community Habilitation, In-home Respite, Bridges to Health, and Traumatic Brain Injury services for an assigned caseload of 50-60 individuals. Responsibilities include assessing individual needs, developing and monitoring service plans, maintaining communication with families and service coordinators, supervising direct care staff, conducting home visits, providing coaching and training to staff, and participating in staff meetings and special projects. Qualifications include a Bachelor's Degree and supervisory experience in home care, strong computer skills, effective communication abilities, and a willingness to travel.
Required Qualifications
- Bachelor's Degree
- 2-3 years supervisory experience in home care field or 4-5 years of direct work experience plus 1-2 years supervisory responsibilities
- Strong computer and statistical reporting skills
- Ability to communicate effectively with a broad range of stakeholders
- Competency in working in fast paced and rapidly changing environment
- Willingness to travel to home sites
