Job Summary
Assess new client needs upon admission and develop initial care plans. Confirm care schedules, follow up with clients, and perform in-person or virtual reassessments to ensure quality care and notify Team Leads of condition changes. Conduct unannounced home visits, perform direct observations of PCAs for annual evaluations, and monitor newly hired staff. Communicate client concerns, prepare incident reports, and maintain positive relationships with existing and prospective clients. Maintain daily schedules in HHAX, complete visits timely, and participate in staff meetings and in-services. Requires a valid driver's license, automobile, and ability to travel daily or weekly in the Allentown area.
Required Qualifications
- High School Diploma
- At least one (1) year of previous experience in health care
- Proficient with Microsoft Office Suite or related software
- Must have the use of an automobile in good working condition
- Valid driver's license
- Automobile insurance compliant with company policies
- Ability to move, transport, or position up to 50 pounds
- Ability to move or traverse about in offices and/or client homes, including ascending and descending stairs
- Ability to communicate effectively and clearly with others to exchange information
- Regular travel on a daily or weekly basis
- Allentown area and surrounding counties
Desired Qualifications
- CNA or HHA certification
- Associate's degree or equivalent industry experience
