Job Summary
Field RN – Per-Diem responsible for creating Care Plans and supervising patient health and wellness per the Care Plan in collaboration with the Nursing Director, physicians, and care team. Administers Care Plans, provides on-site orientations and supervision of home care aides, reviews and updates Plans of Care, conducts nursing assessments, documents services and client status changes, communicates with case managers/families, and travels to patient homes as needed. Requires strong organizational and problem-solving skills, adherence to federal/state regulations, and effective communication with staff, physicians, and families. Travel 10-20% to other markets; on-the-road work environment; per-visit compensation range $75-$100; NYS RN license and valid driver’s license required. Bilingual preferred; CPR preferred; home health experience preferred.
Required Qualifications
- Graduate of an approved school of professional nursing with a Diploma, Baccalaureate, or Master’s Degree
- Currently licensed and registered as a Registered Nurse in the practicing state (NYS required)
- Must be a licensed driver with independent transportation and current insurance coverage
