Job Summary
Field RN (Hospice) per diem role supporting in-home patient care across CA, HI, OR, and WA. Responsible for planning, organizing and directing home care services, managing a patient caseload, delivering direct and indirect skilled nursing care, coordinating with other disciplines, and documenting patient status and outcomes. Must assess patient needs, implement and adjust plans of care, educate patients and families on nursing care and rights, ensure HIPAA-compliant documentation, and participate in on-call duties. Requires RN licensure, CPR/BLS certification, valid driver's license and auto insurance, and compliance with organizational vaccination requirements. Adventist Health-affiliates emphasize on-call duties and ongoing professional conduct, with E-Verify participation.
Required Qualifications
- Registered Nurse (RN) licensure in the state of practice (required)
- CPR or Basic Life Support (BLS) certification (required)
- Current automobile insurance and a reliable automobile (required)
- Valid Driver's License and must be at least 21 years of age (required)
- Education: Bachelor's Degree in Nursing (BSN) preferred (implies higher qualification)
- Acute care facility or home care experience preferred (implies qualification)
- Vaccinations as a condition of employment (required per organizational policy)
