Field RN (Home Health)

Manage patient care and scheduling while monitoring, recording, and reporting symptoms and changes in patients' conditions. Maintain accurate OASIS reports, create 485 assessments, and modify treatment plans based on physician orders and patient responses. Coordinate with healthcare teams to assess, plan, implement, and evaluate care, instructing families on health education and disease prevention. Direct less skilled personnel, supervise nursing care, and interpret diagnostic tests to identify patient conditions. Assess home environments for safety risks and inform physicians of patient status as required. Travel throughout the assigned service area to provide care in patient homes.
