Job Summary
Conduct initial assessments of new clients to determine healthcare needs and develop individualized care plans. Perform regular home visits in the Bronx and Westchester to assess client progress and adjust care plans. Collaborate with the interdisciplinary team, provide education and emotional support to clients and families, and maintain accurate patient records. Deliver in-service trainings and guidance to Home Health Aides and Personal Care Assistants while participating in team meetings to contribute to care strategy development. Requires current NYS RN license, one year of nursing experience, valid driver's license, and reliable transportation.
