Job Summary
Per Diem Pediatric RN Case Manager conducting comprehensive patient assessments (including OASIS data collection) and delivering home-based nursing care. Responsible for coordinating individualized pediatric care plans with physicians, families, and healthcare professionals; directing LPNs and Home Health Aides to ensure quality care; monitoring clinical documentation for accuracy and regulatory compliance; and participating in quality improvement and staff education. Flexible, per-visit scheduling with predictable on-the-road pediatric visits and mileage reimbursement, in a collaborative, autonomous clinical environment.
Required Qualifications
- Active PA RN License (Required)
- 2+ years of professional nursing experience (pediatric or home health experience preferred)
- Strong leadership, communication, and organizational skills
- Ability to manage multiple priorities and work independently
- Valid driver’s license
- Current CPR certification
