Job Summary
RN, Nights and Weekends provides after-hours field visits to patients, families, and community partners, establishing and maintaining positive relationships with team members, patients/families, and referral resources. Responsibilities include making after-hours visits to patients in their homes or facilities, assessing immediate needs, delivering direct nursing care as part of the Interdisciplinary Group, prioritizing care, educating patients/families about disease process and palliative care, coordinating with other professionals, documenting per policy, and ensuring safe discharge and follow-up. The role emphasizes compassionate patient service, clinical competencies in case management, communication via SBAR, and collaboration with admissions and other stakeholders. Compensation range: $67,976.53 - $106,213.33.
Required Qualifications
- Current license as RN in the state where the employee will be working
- Minimum of one year nursing experience; hospice or hospital experience preferred
- Ability to effectively communicate in English (verbal and written); bi-lingual in Spanish preferred
- Mobile Driver - Valid driver’s license and automobile insurance per Company policy
- Able to use EMR/EHR systems
- Excellent clinical and assessment skills
- Ability to work in a fast-paced environment and handle stressful situations calmly
- Ability to perform essential functions including lifting etc. with or without reasonable accommodations
- BLS for healthcare professionals from AHA or Red Cross
