Conduct initial assessments of new clients to determine healthcare needs and develop individualized care plans. Perform regular home visits to assess client progress and adjust care plans as necessary. Collaborate with the interdisciplinary team to ensure proper patient care, while providing education and emotional support to clients and their families. Maintain accurate patient records and documentation. Provide in-service trainings and guidance to Home Health Aides and Personal Care Assistants to ensure high-quality care delivery. Participate in team meetings to contribute to the development of care strategies. This position is based in Brooklyn and requires weekly travel to Queens, Manhattan, and Long Island.
