Job Summary
Conduct in-home assessments for Alignment Health members, including medication reviews and health screenings. Provide patient education by matching desired care with clinical best practices. Identify diagnoses for active medical management and coordinate comprehensive care plans with Regional Medical Officers, PCPs, and specialists. Lead broader clinical teams comprising nurses, health coaches, and social workers to support care management goals. Prevent unnecessary hospitalizations and unmanaged disease progression through timely clinical interventions in the Care Anywhere program.
Required Qualifications
- Master's degree from an accredited NP Program or PA program
- Active Nurse Practitioner or Physician Assistant license
- Active RN License and Furnishing number
- NP only Active Nurse Practitioner Board Certification
- Physician Assistant Certification
- NPI Number
- DEA
- Valid BLS
- Valid Driver License
- Current automobile insurance
- Ability to communicate positively, professionally and effectively with others
- Effective problem solving skills
- Organizational skills
- Time management skills
- Ability to work in a fast-paced environment
- Essential Physical Functions: regularly required to talk or hear

Desired Qualifications
- One (1) year of prior clinical or home care experience
- Previous EMR experience
- Experience in care of older adult (geriatric) patients
