Job Summary
Develop and maintain positive relationships with physicians, members, families, and providers while coordinating activities and referrals to community resources. Perform care plan interventions, conduct screenings at member homes, and monitor outcomes to ensure quality service delivery. Collaborate with the care coordination team to initiate self-management plans and build care plans for STAR Kids Level of Care 3 members. Interpret relevant information with families to improve access and address unmet needs in healthcare, housing, and transportation. Requires a driver's license, five years of healthcare experience, and CHW certification.
Required Qualifications
- H.S. Diploma or GED
- Driver's license in any state
- auto insurance
- 5 years of experience in healthcare, local and state programs for children with disabilities or managed healthcare setting
- Authorization to Release Personal Information must be signed prior to beginning work in this role
Desired Qualifications
- Bachelor's degree
- CHW – Community Health Worker certification by Texas Department of State Health Services (DSHS)
