Field Registered Nurse (RN)

Conduct home health visits to administer IVs, change dressings, clean wounds, and perform comprehensive patient assessments. Document all findings and physician orders in Synergy Software while initiating and revising care plans in consultation with the Clinical Team Manager. Coordinate care with physicians and other team members, manage re-certifications and discharges, and supervise Home Health Aides per regulatory requirements. Work at least two to three days per week with flexibility for weekends and holidays. Requires one year of nursing experience, a current CA RN license, BLS certification, and IV/Phlebotomy credentials.
