Job Summary
Conduct home health visits to administer services, perform comprehensive assessments, and update physician orders for patients in Bakersfield, Lancaster, and surrounding So-Cal areas. Manage patient treatment plans, coordinate care with the Clinical Team Manager and physicians, and document all activities in Synergy Software. Supervise Home Health Aides, handle re-certifications and discharges, and assess needs for additional team members like PT, OT, and ST. Maintain regulatory compliance and communicate significant condition changes to the assigned team. Requires at least one year of RN experience, current CA license, IV/Phlebotomy certification, and BLS. Pay ranges from $50 to $140 per visit with flexible scheduling including weekend and holiday coverage.
Required Qualifications

- Ability to work at least two-three days each week (or ten productivity units)
- Ability to work one weekend a month
- Ability to work two holidays per year (one winter and one summer)
- 1 year of experience as a Visiting RN for Home Health
- 1 year of Nursing experience
- Current CA RN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- IV/Phlebotomy Certified
