Job Summary
Conduct comprehensive home care assessments, including initial patient and family evaluations, physical histories, and ongoing re-evaluations using the OASIS Data Set. Develop and revise Plans of Care that establish goals, incorporate therapeutic actions, and coordinate discharge planning while communicating daily updates to the Clinical Team Manager. Administer prescribed medications, perform preventive and rehabilitative procedures, and provide health teaching regarding disease processes and self-care techniques. Coordinate all patient care activities, document communications with physicians and multidisciplinary teams, and supervise Home Health Aides and Licensed Vocational Nurses. Participate in on-call rotations, including weekends, and maintain daily patient case loads with strict adherence to agency policies and infection control standards.
Required Qualifications
- High school diploma or equivalent
- Graduate of an accredited school of nursing
- Registered Nurse with an active unrestricted license to practice professional nursing in the state of Texas
- Valid Texas Driver's License
- Auto insurance coverage in accordance with organization requirements
- Current CPR certification
- 2+ years of clinical experience preferable in a community home health or medical/surgical setting
- Experience working with electronic medical record application
- Thorough knowledge of current nursing practice, professional standards of care, and state and federal regulations regarding home care
- Bilingual (English/Spanish) language proficiency
- Demonstrated ability to define problem, collect data, establish facts, interpret an extensive variety of technical, medical, regulatory instruction and deal with numerous issues to draw a valid conclusion
- Demonstrated ability to endure prolonged or considerable walking or standing
- Ability to lift position or transfer patients in a proximate location
- Ability to lift supplies and equipment
- Ability to perform reaching, stooping, bending, kneeling, or crouching
- Visual acuity and hearing, functional or corrected, to perform required nursing skills
- Demonstrated ability to prioritize and communicate objectives clearly
- Demonstrated ability to comply with accepted professional standards and practices
- Proven ability to interact productively with individuals and with multidisciplinary teams
- Proven evidence of independent nursing practice in delivering nursing care required
- Proven basic computer skills to include: Microsoft Word, Outlook, and other e-mail systems
- Proven excellent verbal and written skills
- Tuberculosis screening
- Proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease
- Ability to handle stress situations with calmness and courtesy at all time
- Pass a drug test before beginning employment
