Job Summary
Conduct initial and ongoing nursing assessments for clients with SPMI/SUD while carrying a caseload of weekly visits in homeless shelters, client homes, or the community. Develop treatment plans with the team, coordinate medication administration and pharmacy follow-ups, and make referrals to physicians or hospitals as needed. Act as a liaison between external providers, shelters, and the ACT team to facilitate care, deliver health education, and respond to medical emergencies. Perform charting, monitor lab results, and participate in interdisciplinary meetings. Work 80% of the time in the field and 20% in the office, requiring stamina for prolonged standing and walking.
