Job Summary
Nurse Practitioner (NP) in Minot, ND conducting in-home Annual Wellness Visits and chronic condition assessments for Medicare members and at-risk populations as a 1099 PRN contractor. Responsibilities include focused histories, exams, vitals, medication reviews, and screenings (Hemoglobin A1C, Spirometry, KED, DRE, FIT); educating members on preventive care and chronic disease management; documenting visits in EMR; working independently with training and support provided; travel within a 55-mile radius of the assigned county; flexible weekday availability with a minimum commitment of ~30 hours per month; compensation ~$100 per completed visit plus mileage.
Required Qualifications
- Active, unencumbered NP license in the applicable state
- 3+ years of patient care experience preferred
- Willing to obtain additional licensure if needed (supported by Advantmed)
- In-home or risk-adjustment experience preferred
- EMR experience
- Travel within 55-mile radius of assigned home location
