Job Summary
Provide screening, preventive primary care and medical care services to members in non-clinical settings including in-home, community and nursing facilities and pop-up clinics. Responsible for comprehensive evaluations, care coordination across adult, women’s health, pediatric and geriatric populations, ordering and interpreting diagnostic tests, developing medical plans of care, scheduling visits, and coordinating with physicians. Performs face-to-face in-person visits in homes, skilled nursing facilities, and other community sites, with potential travel up to 120 miles daily within the region and occasional overnight travel for special projects. Requires cross-state licensure as needed, and collaboration under physician supervision; strong emphasis on delivering accessible, quality care and effective charting in EMR. Must maintain active licensure, DEA registration, BLS, and valid driver’s license for job-related travel.
Required Qualifications
- At least 1 year of experience as a nurse practitioner, or equivalent combination of relevant education and experience
- Active and unrestricted national certification from American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
- Current state-issued license to practice as a Family Nurse Practitioner (FNP), active and unrestricted in state of practice
- Prescriber Drug Enforcement Administration (DEA) license with authority to prescribe per state qualifications
- Current Basic Life Support (BLS) certification
- Valid and unrestricted driver's license with reliable transportation for job-related travel requirements
- Ability to work within a variety of settings and adjust style as needed
- Ability to work independently with minimal supervision and demonstrate self-motivation
- Strong communication; verbal and written
- Microsoft Office and EMR experience
