Job Summary
Field Nurse Practitioner in the Vancouver, WA area provides screening, preventive primary care and medical care services to members in non-clinical settings such as in-home, community, nursing facilities, and pop-up clinics. Responsibilities include general medical care and care coordination across adult, women’s health, pediatric, and geriatric populations; performing comprehensive history and physicals; addressing chronic and acute primary care needs; establishing diagnoses; coordinating specialty consultations; ordering tests; creating medical plans; scheduling visits; coordinating post-discharge care; conducting face-to-face in-person visits and occasional remote modalities when needed; local travel to multiple sites (up to 120 miles daily) with potential overnight stays; and maintaining cross-state licensure as required. Required qualifications include at least 1 year of NP experience, active national certification (AANP or ANCC), current state Family Nurse Practitioner license, active DEA license, current BLS certification, valid driver’s license and reliable transportation, ability to work independently and with diverse populations, strong communication, time-management, and EMR experience. Preferred qualifications include experience in home health or community/public health settings and bilingual abilities.
