Job Summary
Field Nurse Practitioner providing screening, preventive primary care and medical care services to members, primarily in non-clinical settings (in-home, community and nursing facilities, and pop-up clinics). Responsibilities include comprehensive evaluations (history and physicals), addressing chronic and acute primary care needs, care coordination, establishing medical diagnoses, ordering labs/imaging, coordinating specialty consultations, creating and implementing care plans, scheduling follow-ups, and providing post-discharge coordination. Travel up to 120 miles daily within the assigned region; may obtain cross-state licenses as needed; collaborates with physicians and regional teams; may prescribe medications and perform procedures as appropriate; requires strong communication, time-management, and EMR proficiency.
Required Qualifications
- At least 1 year of experience as a nurse practitioner, or equivalent combination of relevant education and experience.
- Active and unrestricted national certification from AANP or ANCC.
- Current state-issued license to practice as a Family Nurse Practitioner (FNP).
- DEA license with authority to prescribe per state qualifications.
- Current Basic Life Support (BLS) certification.
- Valid and unrestricted driver's license and reliable transportation for job-related travel requirements.
