Job Summary
Field Nurse Practitioner role providing screening, preventive primary care, and medical care in non-clinical settings (in-home, community, nursing facilities, and pop-up clinics). Responsibilities include comprehensive evaluations, chronic/acute care management, care coordination, ordering/performing diagnostic testing, determining medical diagnoses, arranging specialty consultations, developing medical care plans, scheduling visits, providing post-discharge coordination, and conducting face-to-face visits in various settings with potential cross-state licensure. Requires active FNP license, DEA license, BLS, and EMR experience; travel within Florida and cross-state practice as needed. Competitive annual pay range with opportunities for work across multiple community health settings and populations (adult, women’s health, pediatric, and geriatric).
Required Qualifications
- At least 1 year of experience as a nurse practitioner, or equivalent combination of relevant education and experience
- Active and unrestricted national certification from AANP or ANCC
- Current state-issued license to practice as a Family Nurse Practitioner (FNP) with active/unrestricted status
- DEA license with authority to prescribe per state qualifications
- Current Basic Life Support (BLS) certification
- Valid and unrestricted driver's license and reliable transportation for job-related travel
- Ability to work in diverse settings and with diverse populations
- Ability to work independently with minimal supervision
- Strong communication skills (verbal and written)
- Experience with EMR systems
- Cross-state licensure based on business need
- Ability to perform or coordinate preventive primary care, care coordination, and chronic/acute condition management
- Ability to travel within assigned region and drive up to 120 miles per day (as needed)
- Ability to collaborate with physicians and other health care professionals
- Ability to manage time effectively and handle multiple projects
