Field Nurse Practitioner - Orange County, California

Conduct in-home Annual Wellness Visits and chronic condition assessments including histories, exams, vitals, medication reviews, and screenings (A1C, spirometry, DRE, FIT). Educate members on preventive care and chronic disease management. Document visits in EMR to support value-based care and risk adjustment. All assessments are conducted in-home with training and EMR support provided. This is a 1099 PRN role with flexibility and a minimum commitment of ~30 hours per month, traveling within a 55-mile radius of the assigned county. Requires an active NP license, 3+ years of patient care experience preferred, and ability to work independently in the field.
