Job Summary
Conduct in-home Annual Wellness Visits and chronic condition assessments for diverse patient populations within LA County; perform focused histories, exams, vitals, medication reviews, and screenings (Hemoglobin A1C, Spirometry, KED, DRE, FIT); educate members on preventive care and chronic disease management; document visits in the EMR to support value-based care and risk adjustment; 1099 PRN role with consistent monthly availability, travel within a 55-mile radius, and strong operational support; 30 hours per month minimum commitment; flexible scheduling with weekdays available; active NP license in CA required; 3+ years patient care experience preferred; EMR proficiency and willingness to obtain additional licensure if needed; paid per completed visit plus mileage; training and onboarding provided; independent field work with autonomy and professional accountability; in-home visits are standard; local focus on in-home wellness and risk-adjustment assessments.
Required Qualifications
- Active, unencumbered NP license in the applicable state
- 3+ years of patient care experience preferred
- Comfortable with EMR systems and independent work
- In home or risk adjustment experience preferred
- Willing to obtain additional licensure if needed (supported by Advantmed)
- Proficiency in conducting in-home wellness visits and chronic condition assessments
- Ability to travel within a 55-mile radius of the assigned county
- PRN/1099 independent contractor engagement structure
- Excellent documentation and professional communication skills
- Dedication to quality and standardized visits
