Conduct in-home Annual Wellness Visits and chronic condition assessments, including focused histories, exams, vitals, and screenings such as Hemoglobin A1C and Spirometry. Educate members on preventive care and document standardized visits in the EMR to support value-based care and risk adjustment. Maintain monthly availability for 30+ hours, responding to coordinators for scheduling and honoring confirmed assignments within a 55-mile radius. This 1099 PRN role offers ~$100 per visit plus mileage, flexible weekday hours, and immediate visit availability for NPs seeking reliable supplemental income.
Required Qualifications
- Active, unencumbered NP license in the applicable state
- Maintain ongoing monthly availability
- Be responsive during onboarding and scheduling
- Honor confirmed visits and assignments
- Work independently and professionally in the field
- Minimum commitment: ~30 hours per month, ongoing
- Travel within a 55-mile radius of the assigned county
Desired Qualifications
- 3+ years of patient care experience preferred
- Comfortable with EMR systems and independent work
- In home or risk adjustment experience preferred
- Willing to obtain additional licensure if needed
