Job Summary
Conduct structured in-home wellness and risk-adjustment assessments for diverse patient populations, including Annual Wellness Visits and chronic condition evaluations. Perform focused histories, physical exams, vital signs, medication reviews, and specific screenings such as Hemoglobin A1C, Spirometry, KED, DRE, and FIT. Educate members on preventive care and chronic disease management while documenting standardized visits in the EMR to support value-based care and risk adjustment. Maintain ongoing monthly availability and honor confirmed weekday assignments within a 55-mile radius of the assigned county. This 1099 PRN role offers flexible scheduling with paid mileage and dedicated operational support for licensed NPs seeking steady supplemental income.
Required Qualifications
- Active, unencumbered NP license in the applicable state
- Valid unencumbered NP License for the state you will be working in
- Daily travel expectations remain within a 55-mile radius of the assigned home location
- Travel up to a maximum 55-mile radius originating in the assigned job posting county
- Access to reliable transportation that will enable you to travel to members' homes within a designated area
- Strong ability to work within our EMR system
- Ability to work independently
- Must have a valid unencumbered NP License for the state you will be working in

Desired Qualifications
- 3+ years of patient care experience preferred
- Comfortable with EMR systems and independent work
- In-home or risk-adjustment experience preferred
- Previous in-home risk assessment experience preferred
- Previous 1099 (PRN) experience is preferred
- 3 years patient care experience preferred (primary care/adult/geriatric, EMR)
- Preference is given to weekday schedules
- Preference is given to standard working hour schedules
- May be requested to obtain additional NP licensure supported by Advantmed
- Bilingual is a plus
