Job Summary
Conduct comprehensive nursing assessments covering physical, functional, psychosocial, and environmental factors while deriving diagnoses and collaborating with patients, families, and physicians to develop individualized care plans. Deliver direct therapeutic care, ongoing education, and training to preserve patient autonomy and optimize outcomes through evidence-based practice. Coordinate interdisciplinary care to promote patient satisfaction and health goals, while maintaining accurate documentation aligned with Federal and Joint Commission standards. Assist leadership with quality assessment and performance improvement initiatives to enhance corporate goals. Requires a BSN, active NY RN license, and one year of home care experience. Salary range is $110,966.74 - $160,466.74 annually.
Required Qualifications
- BSN
- active RN license
- at least one year of Home Care/Home Health nursing experience
- Current Registered Professional Nurse in New York State
- Bachelor's Degree in Nursing
- Basic Life Support Certification through the American Heart Association
- Fluent written and spoken English language skills
- Excellent communication (both oral and written), interpersonal, customer service skills
- Critical thinking skills
- Working knowledge of computers
- Registered Nurse License-NYS
- Basic Life Support Cert
