Job Summary
Partner with care team members to develop and implement patient-centered clinical plans for adults and seniors with high medical and social complexity. Deliver wrap-around services through home visits, performing key clinical tasks including chronic condition support, wound care, phlebotomy, and medication review. Engage hard-to-reach patients directly at home to ensure continuity of care while providing coverage for team members when needed. This role requires independent home visits, current CPR and BLS certifications, and intermediate to advanced computer skills for electronic health records. Candidates must hold a valid LPN license, possess a clean driving record with five years of licensure, and be fully vaccinated. Base salary ranges from $38.00 to $42.00 per hour with a bonus program, plus full healthcare coverage, 401K, and 15 days of PTO.
Required Qualifications
- Licensed Practical Nurse (LPN) licensure in the state of employment
- Ability to conduct independent home visits
- Experience providing care for the whole person, including caring for adults and seniors with high medical and social complexity
- Current CPR and BLS certifications
- Intermediate to advanced computer and software skills, including Patient Health Exchange Platforms, Electronic Health Records, and Outlook
- Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation
- Proof of liability and property damage insurance on vehicle used
- Licensed for a minimum of 5 years
- No vehicle-related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years
- No more than 2 moving violations or 1 at-fault accident in the last 3 years
- Fully vaccinated and to provide records for validation
